Our Care Process

Step 1: Referral Support Coordinators, provider agencies, or families reach out with a referral for skilled nursing visits or medication administration. We collect the basic information needed to start the process, confirm eligibility, and clarify the member’s immediate needs. We coordinate with DDD and the member’s AHCCCS health plan to ensure the request aligns with covered services.

Step 2: Nurse Visit Plan Our clinical team reviews the referral details, healthcare orders, and the member’s care environment. We develop a visit plan that outlines frequency, tasks within scope, and any safety or communication needs. Once approved, we schedule visits with one of our qualified nurses so the member receives consistent support. We collaborate with the Support Coordinator, provider agencies, and family to make sure everyone understands the plan and expectations.

Step 3: Ongoing Skilled Support Our nurses visit the member in their home to complete skilled tasks, administer medications, monitor health changes, and document each visit according to ADHS and DDD standards. We communicate updates to the care team as needed and help keep everyone informed and aligned. Our goal is steady, safe support that reduces stress for families and providers.
